Program Description
The Problem:
Individuals within the criminal justice system who are diagnosed with psychiatric illness are not offered supportive services, which results in high recidivism rates.
Our Solution:
Provide tools and case management services for individuals with psychiatric illness to ensure they have the resources needed to successfully re-enter society.
How We Do it:
The Transitional Case Management (TCM) program provides enhanced transition planning for incarcerated individuals at Yaphank and Riverhead jails eighteen years of age and older with a documented psychiatric illness.
Prior to individuals’ discharge, Case Managers begin working on a personalized transition plan within the confines of the jails to be put into place upon the individual’s release, at which point they continue working with participants for approximately three months to secure appropriate primary and behavioral healthcare, vocational and educational services, benefits and entitlements assistance, housing referrals, and peer support.
The Case Managers ensure that participants receive appropriate long-term case management services before being discharged from the TCM program to ensure effective re-entry to society.
