Program Description
The Problem:
The healthcare system can be daunting, especially for those facing the immediate consequences of substance abuse, mental illness, or incarceration, who may find themselves bouncing around the medical system with disjointed care, little preventative care, and costly ER and hospital stays.
Our Solution:
Help individuals navigate the medical system to ensure their medical needs are addressed in a comprehensive and cost-efficient manner.
How We Do it:
In partnership with Health Homes Care Coordination throughout New York City and Long Island, our Health Home Care Coordinators work one-on-one with high-risk individuals to better manage their healthcare needs. They discuss current medical concerns and help participants create treatment plans, schedule appointments, resolve insurance problems, develop self-management skills, and make referrals to other medical and social services in the community. The program also addresses the social determinants of health, like housing instability, which often complicate health issues.

The Outcomes of Health Home Care Coordination
Participants working with our Care Coordinators are better able to manage their ongoing health issues by seeing their primary care doctor on a regular basis, having preventative screening tests, and taking medicine as prescribed, which results in a reduction of unnecessary visits to the ER and hospitalizations.
