Senior & Nutrition Services

EAC Network is the largest provider of Meals on Wheels in Nassau County. We act as guardians for seniors, as well as mentally and/or physically incapacitated individuals, who are unable to manage aspects of daily life on their own. Our senior centers provide warm meals, exercise, and social interaction to local seniors.

Community Guardianship

Community Guardianship manages the personal and/or financial affairs of an individual who cannot manage for him/herself due to physical injuries, mental illness, developmental disabilities, Alzheimer’s disease, or other age-related dementia.

Expanded In-home Services for the Elderly Program (EISEP)

EISEP provides case management, in-home, ancillary, and respite services to older adults who are ineligible to receive services through other government-funded programs.

The Surrogate Decision Making Commitee

Surrogate Decision Making Committee (SDMC)

SDMC intervenes on the behalf of individuals who cannot give informed consent due to their physical or mental state to oversee medical decisions and ensure the individual’s best interests.

The Surrogate Decision Making Commitee

Elder Abuse Enhanced Multidisciplinary Team (E-MDT)

E-MDT is made up of professionals from various fields brought together to discuss complex cases of elder abuse and create action plans to offer resources and services to the victim.
