Program Description

The Problem:

Nassau County has over 300,000 seniors, many of whom are frail, living alone, and otherwise vulnerable.  Most desire to continue to live in their home as long as possible, to “age in place,” but they often have trouble managing daily life on their own.  Furthermore, they may not be eligible for certain government programs that could help them, and the cost of assisted living is too much.

Our Solution:

Maintain older adults safely in their home and delay or prevent costly alternatives of hospitalization or institutionalization by connecting them to the appropriate services.

How We Do it:

Across Long Beach, Lido Beach, Point Lookout, Atlantic Beach, Five Towns, and East Rockaway, EISEP Case Managers meet with Nassau County residents 60 years or older and their family to first complete a comprehensive assessment, which examines existing needs for the senior as well as what barriers may be preventing them from remaining independently in their home. Upon determining what services are needed, such as non-medical in-home services, additional case management, non-institutional respite, home-delivered meals, ancillary services, emergency response systems, and more, the Case Managers work with the senior and family to develop a care plan and then implement the appropriate services. EISEP participants are eligible for the program regardless of income and are required to cost-share according to a sliding scale reflecting their income as well as the cost of services they receive, ranging from no-cost to full-cost.
