Home Base Program

Program Overview

EAC Network’s intensive community Home Base Program is an in-home program that works with 10 to 18-year-old youths who are referred by the Suffolk County Probation Department. Youth involved with the Probation Department, including Persons In Need of Supervision (PINS), PINS Diversion, juvenile delinquents, and New York’s Raise the Age (RTA) program, have been identified as high-risk and in need of support services due to substance abuse issues, health-related issues, family conflict, school truancy, and/or delinquent behavior. A referral is required.

Program Description

The Problem:

Teens may need more help than family alone can offer. While families do their best to help and connect with troubled teens, sometimes additional help is necessary. 

Our Solution:

EAC Network’s Home Base Program offers in-home services with immediate contact.  Wrap-around services are provided and include, but are not limited to case management, counseling, mentoring, job assistance and school advocacy, educational and life skill workshops, recreational activities, and parent education.   The wrap-around services team works with the Probation Department and the youth’s family to provide  internal and community- based services. Support is provided for youth and their family in their home, school, and community, to help meet critical needs.

How We Do it:

After a referral is received and enrollment takes place, our Home Base trained professionals provide immediate contact with each family. All visits with case managers and other support services occur within the home.  The team focuses on anger management, parent education, family counseling, relapse prevention, violence prevention, conflict resolution, positive youth development, and self-esteem building.
