Aging and Basic Needs

Aging and Basic Needs Services provide meals, caregiving support, and essential services that help older adults and families remain stable and independent. We aim to meet their fundamental needs so they can live safely and continue thriving within their communities.

The Problem

Many older adults in Nassau County who have difficulty managing daily life may not qualify for other government-funded support programs or be able to afford assisted living.

How We Solve It

EAC Network’s Expanded In-Home Services for the Elderly (EISEP) provides case management and non-medical home care services that help at-risk older adults remain safely and independently in their own homes. Through comprehensive assessments, individualized care plans, and coordinated support services, our case managers help reduce the need for hospitalization or institutional care.

The Problem

Many older adults aged 60 and over face challenges that make it difficult to safely remain in their homes and maintain independence as they age.

How We Solve It

EAC Network’s Geriatric Social Work Services provide in-home assessments and ongoing support for aging adults at risk of decline to help them remain safe and independent. Through individualized care plans, resource coordination, and professional social work services, our team addresses each person’s emotional, social, and practical needs.

The Problem

Many low-income Nassau County households struggle to afford rising heating and utility costs, often forcing families to choose between keeping their homes warm and meeting other basic needs.

How We Solve It

EAC Network’s Home Energy Assistance Program (HEAP) provides financial assistance to eligible Nassau County residents to help cover heating and utility bills during the winter months. Families must complete an application, submit required documentation, and be interviewed to determine eligibility.

The Problem

Many older adults in Nassau County rely on family members for daily care, and those caregivers often experience significant stress and burnout while trying to support their loved ones at home.

How We Solve It

EAC Network’s In-Home Respite Services provides trained in-home companions to care for older adults while their family caregivers take needed time to rest, run errands, or manage other responsibilities. Companions are carefully screened and matched to each family, offering safe, supportive care and social engagement for seniors.

The Problem

Many older adults in Nassau County face food insecurity and may struggle to afford or prepare nutritious meals while also managing limited income and health related challenges.

How We Solve It

EAC Network’s Meals on Wheels Program provides nutritious meals and supportive services to older adults. In addition to meal delivery, volunteers offer friendly visits and social connection, reducing isolation and supporting overall well-being.

The Problem

Many family members provide ongoing care for aging relatives while balancing work, personal responsibilities, and their own health needs. Without adequate support, caregivers can experience stress, burnout, isolation, and difficulty accessing resources that would help them continue providing care.

How We Solve It

EAC Network’s Nassau Family Caregiver Support Program provides information, referral, counseling, support services, and caregiver education to help family members care for older adults in their homes and communities. Through resource coordination, training opportunities, support groups, and respite assistance, the program helps caregivers reduce stress, strengthen caregiving skills, and maintain their own well being while supporting their loved ones.

The Problem

Many low-income families, seniors, and vulnerable households in Nassau County struggle to consistently access enough nutritious food to meet their daily needs.

How We Solve It

EAC Network’s Nutrition & Wellness Pantry provides food assistance to individuals and families experiencing food insecurity. By offering access to essential groceries, the program helps support health and overall well-being.

The Problem

Many older adults experience isolation at home, leading to decreased social interaction, limited physical activity, and a decline in overall health and well-being.

How We Solve It

EAC Network’s Senior Centers & Bethel Lunch Program provide welcoming spaces where older adults can socialize, participate in recreational and educational activities, and enjoy nutritious congregate meals.

As people age, decreased physical activity can negatively impact health, mobility, and overall well-being.

How We Solve It
